Please download and fill out the Membership Application (.PDF)*, print out and sign, then mail or drop it at the Credit Union office together with copy of acceptable forms of identity and eligibility (see bottom of page 2). Please be sure to include initial deposit of minimum $5 for Share Account (Do not mail cash). Customer Identification Program Policy (2nd page of form) is just for your information and you don’t have to return it to us.
